IRS form 1095-C to be distributed

You may need this medical plan document when filing your 2020 taxes

In late February, the university will be mailing a tax form called the 1095-C to faculty and staff, who may need this form when filing taxes for 2020. The form documents eligibility and/or enrollment in the JHU medical plans for 2020. This same information will be reported to the IRS.

The form is not required to be attached to the form 1040. It is one of many documents (such as insurance cards and statements) that can be given to tax preparers, if needed, to prove that you, or a member of your family, had health care coverage in 2020.
