About Us
Charm City Clinic Inc. was founded in the fall of 2009 by a group of students and community leaders in East Baltimore. The clinic operates within the Men and Families Center, a longtime holistic community services provider that empowers individuals and families in Baltimore City. We believe that health is a human right.
Following in the footsteps of East Baltimore community leaders committed to improving the health of neighbors, we accompany residents of low-income communities in Baltimore through the process of attaining and sustaining access to high-quality health care and other supportive services. We build robust partnerships with our clients—our neighbors—to help them advance their aspirations for well-being and to develop community-based approaches for addressing the social and economic determinants of health and disease in the context of accessing care.
Volunteer Position Description
Health Resource Center: Case Managers
HRC case managers accompany low-income Baltimore residents through the difficult process of accessing sustainable, high-quality health care and other social services. They provide assistance with enrollment in health insurance programs, finding solutions for primary medical care, mental health treatment, and substance abuse disorders, and stabilizing social and economic barriers to care through means-tested benefits and other community resources such as food stamps, energy bill assistance, and job training. Case managers maintain longitudinal relationships with their clients and, in line with the mission of our partner, the Men and Families Center, meet clients "where they're at" by following up at the clinic, by phone, by home visit, in the neighborhood—whatever it takes to help them meet their goals.
